Existing Client?: If you are a current client and would like to schedule or change an appointment with your psychologist or view your account, you may log in to the client portal and manage your appointments and account online, or call us at (239) 561-9955.

REFERRALS:  If you would like to refer a patient, please fax a referral to us at (239) 561-9779. You may also direct your patient to complete the contact form of the doctor you are referring them to. MEDICAL INFORMATION REQUESTS: All requests for contact, appointment, or clinical information must be PRE-authorized by written consent of the patient/client. If you are in need of Protected Health Information (PHI), please complete the Release of Protected Health Information Form  and fax it to (239) 561-9779 or email it to [email protected] and we'll contact you as soon as possible.
