New Client Appointments
NOTE: Please see the "Our Team" page and click the doctor you are interested in to see when they are accepting new patients. You may complete a contact request form located on each doctor's bio page and a staff member will contact you.
Or you may call us at (239)561-9955 to speak with our office staff who will assist you with scheduling an appointment for an initial consultation. If you are greeted by our voicemail recording, please listen to the message carefully and leave a voice message for your call to be returned as soon as possible.
It's important that the doctor you choose to consult with also be compatible with your needs and your schedule. Please review our doctors' profiles on the "Our Team" page for specialties, availability and hours of operation. You may also request to be contacted via our contact forms on each doctors bio page.
When scheduling your first appointment, we will ask for your email address in order to provide you with a secure link to our patient portal in order to streamline your check-in process. You will be asked to complete all documentation at least 3 business days prior to your initial appointment. (If these documents are not completed in time, your appointment may be cancelled or rescheduled.)
What to Bring
Please arrive at least 10 minutes prior to your appointment time and bring your photo id, insurance card (if applicable) and a form of payment (we accept all credit cards, health savings account cards, personal checks and cash).
Late or Missed Appointments
Your 45-50 minute session is dedicated to you. If you are running late, please call us and let us know, as we will consider you a “no-show” after 20 minutes. Our policy requires each client to call 24 hours in advance to cancel or reschedule a session. Otherwise, you may be charged a fee. We all know that "life" happens and will make every effort to reschedule appointments missed due to unforeseen circumstances or illness. Please review the Service Agreement for details about this policy.
